Sugar-free versus sugar in a lozenge

A hard lozenge has to be made of something, and in most of them that something is a sweetener. What the labels actually say.

The sweetener is the lozenge

A menthol cough drop contains a few milligrams of menthol. The lozenge weighs a couple of grams. Almost all of the rest is the base it is made from, and in a traditional hard-candy lozenge the base is sugar and glucose syrup boiled together. In a sugar-free one it is a sugar alcohol, usually isomalt, doing the same structural job.

So "sugar-free" on a lozenge is not a minor reformulation. It is a different confection with the same drug in it.

A product listing more than one sweetener is counted in every row that applies, so the shares do not sum to 100. This counts what the label says, and does not infer a sweetener from anything else.

The high-intensity sweeteners appear on more labels than sugar does, which is not the same as saying most lozenges are sugar-free. A high-intensity sweetener is used in fractions of a percent and is often added to a sugar base as well as to a sugar-free one, to carry the flavour. The structural question is which of sucrose or a sugar alcohol is doing the bulk work.

Sugar alcohols

Isomalt, sorbitol, mannitol and xylitol are sugar alcohols, also called polyols. They are the reason a sugar-free lozenge can be a hard, clear, slow-dissolving object rather than a compressed powder.

Labels that use them frequently carry their own warning about the effect of eating a lot of them, and that warning is printed on the package. This site quotes the warnings section of each label on its product page and does not summarise it, because the wording and the quantity it refers to differ between products.

Reading the package

The sweetener is in the inactive ingredient line, not in the Drug Facts panel's active ingredient line. If a lozenge matters to you for a dietary reason rather than a pharmacological one, that is the line to read, and each product page here quotes it as filed.
